This form is to request assistance from the District 96 Custodial Team. While it is tempting to ask the custodians to help whenever you see them, verbal requests can often be forgotten in the hustle and bustle of a packed school day. Please take advantage of this process to ensure that our custodians can assist you in the most efficient manner possible. 

Custodial Request Examples: 

• There is an empty soap dispenser in the Staff washroom across the hall from room 708. Can you please fill this dispenser? 
• Please deliver 10 boxes from Mr. Smiths classroom (room 202), to Woodlawn room 800. Boxes are labeled Mr. Smith 1-10. 
• Can you please install the new banner in the Large Gym for the assembly next Month? Please see Mr. Smith for more details. 
• The window in my classroom is cracked. Can you please take a look to make sure it is secure and have it replaced? 

If there is a safety issue that could cause harm please report it your school office so that it can be addressed immediately. 

Please note, not all service requested will be approved. Each request will be reviewed prior to approval to ensure that it aligns with best practice and the District's safety standards.
